So I found a twitter thread talking about this, and the documentation page seems to indicate it’s used as a lookup for a name to indicate the most likely gender of that name is. https://pecl.php.net/package/gender/0.6.0

The Fediverse is imperfectly connected, unfortunately, so you’re not guaranteed to see everything that exists on your instance. Hopefully features will be introduced to avoid this exact duplication problem.
From the documentation, it appears that the country codes are for localization presumbly of the names of the genders.
