I also kinda wonder why Lemmy displays the preview sometimes, but not always. Maybe it has to do with image size? Or some metadata? My portfolio uses the non-standard og:image meta tag that usually works.
So there are multiple internet protocols for displaying preview images, I’m not sure which ones Lemmy supports. Open graph is fairly common these days, if it doesn’t support it yet, could submit a request.
