Do not recommend that people tell their provider to document refusals. Many providers have admitted to viciously retaliating against patients who do this by putting things such as “malingering” in their charts or getting patients banned from medical systems.

A possible valid alternative is (politely) messaging providers after the appointment to go over what was discussed. That becomes your “paper trail”.
