    “Out of pocket” in this context is a reference to a type of patient payment. Healthcare providers charge more money for services to insurance than they charge to patients who pay up front, which would be fine if it was a small amount, but with some insurances you might end up paying more out of pocket with insurance coverage covering part of the visit.
